The Cancer Wellness Program is a supportive care program designed for individuals with cancer who are moving from clinical treatment to survivorship. It assists cancer survivors to make the change from clinical management to supported self-management. The program is run by trained health professionals and supported by Cancer Council Victoria. Topics covered include adjusting to physical and emotional changes after cancer treatment; working with a GP to build a healthy lifestyle; returning to work; managing the financial impacts of cancer; and connecting to support services in the community. It also includes the opportunity to share stories and tips with others going through similar experiences.The Cancer Wellness Program is held in most public and private Melbourne hospitals, some community health centres and many country hospitals and health services.
