DQES v2
The Dietary Questionnaire for Epidemiological Studies Version 2 (DQES v2) is a modification of a food frequency questionnaire (FFQ) that was developed by Cancer Council Victoria in the late 1980s to measure dietary intake of people taking part in the Melbourne Collaborative Cohort Study (MCCS). These were men and women, recruited for the study between 1990 and 1994, who at the time of recruitment were 40-69 years of age and resided in Melbourne, Victoria. The majority were born in Australia, Greece or Italy.
The DQES v2 covers five types of dietary intake, incorporating 80 items:
- cereal foods, sweets and snacks
- dairy products, meats and fish
- fruit
- vegetables
- alcoholic beverages

DQES v2 format
DQES v2 is available in both electronic (online) and hardcopy (printed) formats.
Electronic
The electronic version of DQES v2 provides a more economical option; as the collection of electronic data and its analysis can be performed more efficiently than with paper based systems. This difference is reflected in the lower unit price for electronic DQES v2. Electronic DQES v2 is completed online through a secure server. The online questionnaire takes participants about 5 minutes to complete.
Hardcopy
For those participants for whom online completion is not feasible, a printed version of DQES v2 is also available. Printed questionnaires are posted to the Institution and competed questionnaires are to be returned to Cancer Council Victoria for analysis.
Analysis of DQES v2
Analysis of questionnaires for assessment of dietary intakes is undertaken by the Nutritional Assessment Office, Cancer Council Victoria, and is included in the service fee.
The output of the DQES v2 analysis includes:
Processing, analysis and reporting for both the electronic and hardcopy versions of DQES v2 takes a minimum of two weeks.
Administering DQES v2
DQES v2 is designed to be self-administered, but can also be interviewer-administered.
If self-administered, it is the responsibility of the researcher to ensure that the questionnaires have been completed correctly, with no pages or questions left blank, before returning them to Cancer Council Victoria for analysis.

Fees & ordering
The DQES v2 is made available for use by other parties on a fee-for-service basis, but it remains the property of Cancer Council Victoria.
The price per unit (as detailed in Table 1) includes questionnaires, analysis, postage and GST.
Please note: There is a minimum order size of 55 questionnaires.

Login details for online questionnaires or copies of printed questionnaires are dispatched within a week of receiving an order.
Refunds are not available for any questionnaires that are not used.
Invoicing
Payment for DQES v2 can be made with credit card or by invoice. Invoices and receipts for credit card payments are emailed by our Finance Department once the request or payment is received and processed.
