New home for the Care Plan for the Dying Person – Victoria (CPDP-Vic) resources

On July 1st, the Centre for Palliative Care handed over the future management of the CPDP-Vic clinical resources to the Palliative Care Clinical Network at Safer Care Victoria. This follows a successful state-wide launch of the resources to the sector during 2017.

  

The Centre for Palliative Care through the Victorian End-of-Life Care Coordinating (VEC) Program has had a long association with supporting the care of people recognised to be imminently dying in Victorian health services.

 

The suite of CPDP-Vic resources, which include a multidisciplinary care plan and consumer brochure, were developed with funding from the Department of Health and Human Services.
