Advance Care Planning Australia is a national program that enables Australians to make the best choices for their future health and care, by providing information, support and education to health professionals, aged and community care workers and the general public. The program offers free resources to assist in creating an advance care plan and runs a national advisory service, whereby specialist trained volunteers provide advice to callers. Funded by the Australian Government Department of Health and Aged Care, Advance Care Planning Australia is administered via Austin Health.
